ef fects of the valsalva maneuver and head rotation on internal jugular vein diameter and location by ultrasonography

چکیده
in order to determine the effect of valsalva maneuver and head rotation on the internal jugular vein (ijv) diameter and cross-sectional area and overlap with the carotid artery (ca) and find the best technique for safe cannulation of the ijv and decrease the risk of ca puncture, the diameter of both the ijv and the ca and percentage of overlap between the two vessels and cross-sectional area of the ijv were measured by ultrasonography on the right side of the neck in the supine, head down position, at three different degrees of head rotation with and without the valsalva maneuver in 30 subjects. the results were analyzed by two-way repeated- measures analysis of variance followed by least significant difference (lsd). head rotation increased the overlap between the two vessels (p<0.001). valsalva's maneuver also increased overlap between the twp vessels (p<0.02) and increased ijv diameter and cross-sectional area (p<0.001). head rotation did not change ijv diameter and cross-sectional area significantly (p>0.05). valsalva's maneuver and head rotation did not change ca diameter significantly (p>0.05). we therefore advocate the neutral head position with valsalva's maneuver as a safe and reliable method for ijv cannulation.

EF FECTS OF THE VALSALVA MANEUVER AND HEAD ROTATION ON INTERNAL JUGULAR VEIN DIAMETER AND LOCATION BY ULTRASONOGRAPHY
In order to determine the effect of Valsalva maneuver and head rotation on the internal jugular vein (IJV) diameter and cross-sectional area and overlap with the carotid artery (CA) and find the best technique for safe cannulation of the IJV and decrease the risk of CA puncture, the diameter of both the IJV and the CA and percentage of overlap between the two vessels and cross-sectional are...
متن کاملComparison of internal jugular vein dilation between Valsalva maneuver and proximal internal jugular vein compression
OBJECTIVE The Valsalva maneuver is recognized as an effective method to dilate the internal jugular vein (IJV). However, this maneuver cannot be performed in many cases, such as children and unconscious patients. The aim of this study was to evaluate the effectiveness of proximal IJV compression, which can easily be performed, regardless of patient cooperation. METHODS Healthy adult volunteer...
متن کاملHead rotation during internal jugular vein cannulation and the risk of carotid artery puncture.
We undertook a prospective laboratory study to examine the effect of head position on the relative positions of the carotid artery and the internal jugular vein (IJV). Volunteers (n = 12) from departmental staff, 18-60 yr of age, who had never undergone cannulation of the IJV underwent imaging of their IJV and carotid artery. With the subject in a 15 degrees Trendelenburg position, two-dimensio...
متن کاملRelationship between Trendelenburg tilt and internal jugular vein diameter.
OBJECTIVES To evaluate the relationship between Trendelenburg tilt and internal jugular vein (IJV) diameter, and to examine any cumulative effects of tilt on the IJV diameter. METHODS Using a tilt table, healthy volunteers were randomised to Trendelenburg tilts of 10 degrees, 15 degrees, 20 degrees, 25 degrees, and 30 degrees. Ultrasound was used to measure and record the lateral diameter of ...
متن کاملOptimal head rotation and puncture site for internal jugular vein cannulation after laryngeal mask airway insertion.
INTRODUCTION We studied the effect of head rotation on the relative position of the right common carotid artery (CCA) and the right internal jugular vein (IJV) in patients with laryngeal mask airway (LMA) insertion to evaluate the accuracy of anatomical landmarks for right IJV cannulation. METHODS We simulated needle insertion to the right IJV on sonograms via the central landmark approach an...
متن کاملA failure of internal jugular vein catheterization caused by the internal jugular vein occlusion
provided the original work is properly cited. CC Central venous catheterization is frequently used as the procedure for preparation of major surgery, chemotherapy, total parenteral nutrition, and etc [1]. Usually, the catheterization sites are femoral vein, subclavian vein, and internal jugular vein (IJV). Because the right IJV has easy accessibility, and it gives direct route for right atrium ...
